GREEN BAY, WI (WTAQ-WLUK) — A community survey is hoping to get a pulse on the health goals of Brown County.
Beyond Health is a group made up from different health agencies and organizations in the county.
On Tuesday, the group encouraged Brown County residents to participate in its new Community Health Assessment (CHA) survey.
The answers from the survey will be featured in the upcoming Community Health Assessment, a report that provides a snapshot of the community’s health and serves as the basis for future health priorities.
“A Community Health Assessment is like a doctor’s checkup for the entire community,” said Chrystal Woller, Health Director/Officer for the City of De Pere. “It helps identify our most important health needs so local organizations can make informed decisions about programs, services, and resources.”
The priorities determined by the Community Health Assessment, or CHA, will become the focus of a Community Health Improvement Plan, or CHIP. These two documents work together in a cycle that Beyond Health updates every three years. This keeps community-level health on a path of continuous improvement.
The survey is confidential, and open to anyone in Brown County—age 18 and over—until the end of September, in print or online.

Paper copies of the survey are available at all Brown County Library locations and Brown County Public Health’s Green Bay office (111 N. Jefferson Street).
Those who take the survey can enter for a chance to win one of twenty-five $50 gift cards that will be drawn at random. Survey participants are not required to enter this drawing.
